Aetna, BCBS PPO, Cigna, and Medicare insurances are accepted.  The limitations of coverage and benefits will be verified and explained ahead of appointments. Credit card payments are accepted for self-pay evaluations.

Directions

The office is located about 10 miles west of downtown Chicago, is accessible from the CTA elevated train (Green Line “L” at Oak Park), Metra line (Harlem/Marion Station).

From downtown Chicago:
Head west on I-290, exit 23A Austin Avenue, turn right at Austin Avenue, turn left at Madison Avenue, turn right at Oak Park Avenue, continue to 137 North Oak Park Avenue (Oak Park Avenue and Lake Street) or follow directions for parking.

From the west:
Head east on I-290, exit 23A Austin Avenue, turn left at Austin Avenue, turn left at Madison Avenue, turn right at Oak Park Avenue, continue to 137 North Oak Park Avenue (Oak Park Avenue and Lake Street) or follow directions for parking.

From the north:
Take I-90 towards Chicago, exit 81A towards IL-43 Harlem Avenue. Follow signs for Harlem Avenue and continue south, take a left at IL-64 North Avenue, take a right at Oak Park Avenue and continue to 137 North Oak Park Avenue (Oak Park Avenue and Lake Street) or follow directions for parking.

From the south:
Take I-94 West towards Chicago, exit 51H to merge on to the Eisenhower expressway/I-290W, exit 23A Austin Avenue, turn right at Austin Avenue, turn left at Madison Avenue, turn right at Oak Park Avenue, continue to 137 North Oak Park Avenue (Oak Park Avenue and Lake Street) or follow directions for parking.

Parking is available at the Avenue Parking Garage Village of Oak Park:
From Oak Park Avenue, head East on North Boulevard Downtown Garage. First hour is free, then $1/hour.
